To streamline your first visit, we’ll send you an electronic patient intake form via email once your appointment is scheduled. We encourage you to fill out this form online using the link provided. Completing this form in advance helps us get a comprehensive view of your medical history, as certain health conditions and medications can influence your oral health and our approach to your treatment.

Front Desk
When you arrive, our friendly front desk team will welcome you. If you haven’t had a chance to fill out the patient intake form electronically, no worries! We can provide you with a paper form to complete right here in our office. This step is crucial in helping us select the best treatment plan tailored specifically for you.

If you’d like to know more about our recommended treatments and how to best manage your insurance policy, we’ll give you the answers you need. Diagnostics
At Clark Orthodontics, new patients receive complimentary x-rays, photos, and a clinical exam. We have the latest top-of-the-line equipment to make visits and treatment the very best they can be. And if you’re an Invisalign candidate … we’ll use an Itero scanner – a tool used to capture an extremely accurate model of your teeth that will be the base of your treatment … helping us achieve the very best outcome for you.
